80/20 PPO, $600 deductible per person, $1,100 max for family $20 co-pay for office visits (if a part of the PPO network)

$15 co-pay for generic $35 or 30% co-pay for name brand.
90 day supply mail order $25 for generic $70 for name brand

  • $50 Deductible
